Request Student Conduct Record

The Office of Student Conduct & Community Standards provides students with access to their disciplinary and non-academic disciplinary education records.

The Family Educational Rights and Privacy Act (FERPA) gives students certain rights over their education records, including the right to review, challenge, and control the disclosure of their records. For more information, visit the Brown University Registrar's Office.

Disciplinary Records

Use the Disciplinary Information Request form to request disciplinary records for yourself or others (e.g., employers or admissions). Only cases where a student was found responsible with probation or higher status are disclosed, including incident date, violated policies, community status, and completion of terms.

If you have a record request form that needs to be filled out before you can attend medical school, a health professions school, law school, or be admitted into a state bar association, your academic and student conduct records must be checked. For programs in health professions including medical school, please email your forms to hco@brown.edu. For law school or state bar associations, please email your forms to prelaw@brown.edu.

Non-Academic Disciplinary Records

Use the Consent for Release of Non-Academic Disciplinary Records form to authorize the release of your non-academic disciplinary record to others. The form must be fully completed and signed, and records will be released within 45 days or the specified duration. Incomplete forms will not be processed.

Academic Records and Verifications

The Office of the Registrar serves as the custodian for all credit-bearing academic records for all current and former Brown University students.
